Debug Target property
When this property is set to Server, the debugger attaches to the process Ax32Serv.exe.

If you do not set the Debug Target property, you must manually attach Visual Studio to the correct process.

То есть вполне может быть уже присоединен из-за Project/Debug Target = Server.
Как вариант.
